If this sounds like the type of practice you want to be part of, there are a few steps that are required to begin the process of becoming a patient here at KFPC:

Download, print, and complete the Medical Release Form
Provide the completed form to your previous PCP
Ensure the previous PCP faxes your complete medical records to 855.538.3130

Once we have received and reviewed your medical records from your previous PCP, you will receive a second email with directions to complete your enrollment as a patient.
